Position Summary and Purpose:

The LPN is responsible for patient care through the nursing process, collaborating with the interdisciplinary team to ensure effective and safe care delivery, promoting patient recovery and functional improvement.

Essential Functions:

• Demonstrates nursing knowledge within the scope defined by the Kentucky Board of Nursing.
• Completes comprehensive daily assessments and documents patient progress.
• Prioritizes care based on patient needs and reviews care plans as necessary.
• Delivers care according to physician orders and ensures accurate documentation.
• Communicates effectively with team members, reporting patient status and changes promptly.

Education and Experience:

• Completion of an accredited LPN program is required.
• Prior acute care or rehab experience is preferred.

Licensure and Certification:

• Current LPN license in Kentucky is required.
• CPR/BLS certification required upon hire.
