An exciting opportunity has arisen for an Advanced Nurse Practitioner to be part of the new Hospital at Home team in Inverness. Our Hospital at Home program is designed to deliver comprehensive, patient-centred care, reducing the need for hospital admissions and enhancing patient outcomes.

Service Description

This service will provide acute, hospital-level care to people in their homes experiencing conditions that would otherwise require inpatient care. The delivery of Hospital at Home enables a better person-centred care experience, minimizing the risks of healthcare-acquired infections and long hospital stays.

Support Team

The specialist practitioners will be supported by Senior Decision Makers and acute Consultants. They will collaborate with the Integrated Team comprising AHP, Social Work, and Care at Home colleagues. This team is part of the Intermediate Care options available to the Inverness community.

Job Responsibilities

• Deliver hospital-level nursing care to patients in their homes.
• Administer medications, perform diagnostic tests, and manage medical equipment.
• Collaborate with a multidisciplinary team to develop and implement individualized care plans.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and their families on managing health conditions at home.
• Maintain accurate and timely patient records.

Qualifications

• Registered Nurse (RN) with a valid nursing license in Scotland.
• Experience in acute care, emergency care, or home health nursing.
• Strong clinical skills and ability to work independently.
• Excellent communication and interpersonal skills.
• Ability to travel to patients' homes within the designated area.
• Compassionate, patient-focused approach to care.
